Schedule with a Physician

SportsMedicine GRANT & Orthopaedic Associates is dedicated and committed to providing easy access and a smooth process for anyone to schedule with one of our orthopaedic specialty physicians and we will do our very best to accommodate your needs.

To make a referral to one of our orthopaedic specialty physicians, just call our central scheduling at (614) 461-8174.  We will gather all the information necessary to get your patient scheduled. In order to speed up the process, please download and fill out the form below with the necessary information…we will need this information for you to SCHEDULE an appointment for your patient with us.

  • Patient Demographics
      Insurance information (We will need DOB / SSN of Main Subscriber)
      Insurance will be verified and approved
      Prescription / Physician Referral (if applicable)
      Employer or High School Affiliation (if applicable)
Please download the following forms and give them to your patient BEFORE they come in for their first appointment with us, this will speed up the registration process.
When they arrive at our office, they will also be required fill out three additional forms in person that include:
  • SMGOA HIPAA Notice
  • SMGOA Payment Policy
  • Referring Physician Form

You can also request a specific physician, day, date and general timeframe and we will help accommodate you the best that we can.  Keep in mind, this is only a request and that the actual appointment will depend on several factors.
